When we receive your form in our office, we will check it against the sign in sheets and registration list. After this is done, we will submit your hours to the AOA.  A completed attestation form and post course evaluations are required to receive CME credit and a certificate of attendance.

Remember! You can earn additional CME by completing the Outcomes Evaluation Surveys.

Please fax your attestation to 660-627-2623.

Physicians should only claim credit commensurate with the extent of their participation in the activity.

As a *NEW* benefit to AOCD membership we will also track your CME. Any program you attend where CME is earned can be submitted to the AOCD and we will maintain your records.  Please note, we may not be able to report your CME to all agencies.   Report your CME to the AOCD!
